PATIENT PORTAL

Nephrology Associates of Utah utilizes Acumen 2.0, Powered by EPIC. My Chart allows you to send messages to your doctor, view your test results, renew your prescriptions, access your medical records and more. 

Getting Started

There are several ways to sign up for MyChart:

  • Letter - Ask your doctor's office to mail you a letter with instructions on how to sign up.
  • Email or text - Ask your doctor's office to send you a sign-up email or text.
  • Online - Visit myhealthchart.com.
  • At the clinic - Ask a team member to help you sign up during your visit.
In each instance, you'll receive an access code that's valid for a certain period of time.  You'll need this code to open your account.  Then you'll create your own ID and password to use when signing in to your MyChart account.
USE THE PATIENT PORTAL TO:
  • MANAGE YOUR APPOINTMENTS
    • View the details of your past and upcoming appointments. Request a new appointment or cancel a future one.
  • LET A LOVED ONE HELP MANAGE YOUR CARE
    • When you provide full proxy access, family or loved ones can help manage your care.
  • ACCESS TEST RESULTS
    • No more waiting for a phone call or letter. View your results and your doctor’s comments as soon as they are available.
  • COMMUNICATE WITH THE FULL CARE TEAM
    • Get answers to your medical questions without phone tag or unnecessary appointments.
  • PAY BILLS ONLINE
    • View your invoices, and pay your copay and balance online. (Varies by practice)
  • REQUEST PRESCRIPTION REFILLS
    • Send a refill request for your medications.
